If you are ever unable to make your own healthcare choices or to clearly communicate your wishes, other people will have to make choices for you. Advance Directives are a way to let your family, friends, and healthcare providers know, in advance, of your wishes to receive, or not receive medical care and treatment.
To make sure that your family, friends, and healthcare providers understand your choices, it is important to have your Advance Directives in writing. At your visit, you will be asked if you have Advance Directives; we also have forms and more information available if you need.
